Insurance / Payment:

 As a courtesy to our patients, we will bill your insurance directly. We accept most private insurances and are preferred providers for many health insurances.
Some companies we commonly work with are:

           -Blue Shield PPO   -Cigna    -Health Net PPO -United Health Care  
           -Pacific Care PPO -Worker’s Compensation     -and many more....

          We are a Medicare Participating Provider

We also take motor vehicle accident coverage and accept attorney liens. Please call our office at 415.295.8452 or check with your insurance carrier to verify your benefits.
We have special rates for Meridian Sports Club members who choose to pay out of pocket for their physical therapy.
